Water is the main culprit for most wall issues. It can weaken retaining walls by washing out the base material used to support the wall but much more likely it's because of poor grading and improper drainage. Water builds up behind the wall creating great amounts of pressure by saturating the soil. This is why walls lean, bulge and topple over.

Proper compaction is important to overall strength of the wall as well as the proper backfill. Sand or gravel is best providing the drainage needed. A reinforcing grid may also be needed.
